Welcome to Community Healthcare Systems. These Terms of Service ("Terms") govern your use of our website and the early intervention therapy services we provide. By accessing our website or receiving our services, you agree to be bound by these Terms.
If you do not agree with any part of these Terms, please do not use our website or services. If you are accessing our services on behalf of your child, by using our services, you represent that you have the legal authority to consent to treatment on behalf of your child.
These Terms should be read in conjunction with our Privacy Policy and HIPAA Notice of Privacy Practices, which are incorporated by reference.
Description of Services
Community Healthcare Systems provides comprehensive early intervention therapy services for children from birth to age three who have developmental delays or disabilities. Our services include:
- Speech-Language Therapy: Addressing communication, speech, and language development
- Occupational Therapy: Supporting fine motor skills, sensory processing, and daily living activities
- Physical Therapy: Promoting gross motor development, strength, and coordination
- Developmental Intervention: Comprehensive support across all developmental domains
- Family Support Services: Coaching and education to help families support their child's development
Services are provided in your home, at our center locations, in community settings such as daycare, or through secure telehealth platforms, as determined by your child's Individualized Family Service Plan (IFSP) and in accordance with New Jersey Early Intervention System guidelines.
We are a provider within the New Jersey Early Intervention System and comply with all applicable state and federal regulations governing early intervention services.
Eligibility and Enrollment
To receive our services, your child must:
- Be under three years of age
- Reside in New Jersey
- Have a documented developmental delay or diagnosed condition as determined through appropriate assessment
- Be enrolled in the New Jersey Early Intervention System (or in the process of enrollment)
Enrollment in our services requires completion of intake paperwork, consent forms, and development of an IFSP in partnership with your family and the early intervention team.
Your Responsibilities
As a parent or legal guardian receiving services from Community Healthcare Systems, you agree to:
Provide Accurate Information
- Supply complete and accurate information about your child's medical history, development, and needs
- Notify us promptly of any changes to contact information or your child's health status
- Provide truthful information about your family's circumstances when completing intake forms
Participate Actively in Services
- Be present and engaged during therapy sessions
- Implement strategies and activities recommended by therapists between sessions
- Communicate openly with your child's therapy team about progress, concerns, and goals
- Attend IFSP meetings and participate in treatment planning
Respect Scheduling and Cancellation Policies
- Provide at least 24 hours' notice for appointment cancellations when possible
- Be available at the scheduled time and location for therapy sessions
- Understand that excessive cancellations or no-shows may affect service continuity
Maintain a Safe Environment
- Ensure a safe, clean environment for home-based therapy sessions
- Inform therapists of any household hazards, pets, or safety concerns
- Treat all therapists and staff with respect and professionalism
Acceptable Website Use
When using our website, you agree not to:
- Attempt to gain unauthorized access to any systems, data, or accounts
- Use automated tools to scrape, collect, or download content from the website
- Transmit any viruses, malware, or harmful code
- Use the website in any way that could damage, disable, or impair our services

- You will participate in sessions from a private, quiet location free from distractions
- Telehealth may not be appropriate for all situations or all children
- Technical difficulties may occasionally interrupt sessions
- The therapist may determine that in-person services are more appropriate and recommend a change in service delivery
All telehealth sessions are conducted using HIPAA-compliant, encrypted video platforms to protect your privacy and confidentiality.

Professional Services Disclaimer
Our services are provided by licensed, qualified professionals who follow evidence-based practices. However, we cannot guarantee specific outcomes or results. Every child develops at their own pace, and progress varies based on individual factors including the nature and severity of delays, family participation, and other environmental factors.

Termination of Services
Either party may terminate services under the following circumstances:

You may discontinue services at any time by providing written or verbal notice to your service coordinator or therapist. We will provide transition support and documentation as needed.
Our Right to Terminate
We reserve the right to discontinue services if:
- Your child no longer meets eligibility criteria for early intervention
- You fail to comply with program requirements or these Terms
- There is a pattern of excessive cancellations or no-shows
- Non-payment of applicable fees continues after reasonable notice
- Threatening, abusive, or unsafe behavior toward staff occurs
- Clinical judgment determines our services are no longer appropriate or beneficial
We will provide written notice and assist with transition to alternative providers when appropriate.
Governing Law and Jurisdiction
These Terms shall be governed by and construed in accordance with the laws of the State of New Jersey, without regard to its conflict of law provisions. You agree that any legal action or proceeding arising out of or relating to these Terms or our services shall be brought exclusively in the state or federal courts located in Essex County, New Jersey.
Our services are also subject to all applicable federal regulations, including IDEA Part C (Individuals with Disabilities Education Act), HIPAA, and FERPA, as well as New Jersey Early Intervention System regulations.
Dispute Resolution
If you have concerns or disputes regarding our services, we encourage you to first contact our Clinical Director or Program Administrator to seek informal resolution. Most concerns can be resolved through open communication.
As a participant in the New Jersey Early Intervention System, you also have rights under federal and state law to request mediation or file formal complaints. Information about these rights and procedures is provided in your Parent Rights document and is available upon request.
Indemnification
You agree to indemnify, defend, and hold harmless Community Healthcare Systems and its employees, contractors, and agents from any claims, losses, damages, liabilities, and expenses (including reasonable attorneys' fees) arising from your violation of these Terms or misuse of our website or services.
Severability
If any provision of these Terms is found to be invalid, illegal, or unenforceable, the remaining provisions shall continue in full force and effect. The invalid provision shall be modified to the minimum extent necessary to make it valid and enforceable.
Changes to These Terms
We reserve the right to modify these Terms at any time. When we make changes, we will:

- Notify current service recipients of material changes by email or written notice
