Understanding the consent process for early intervention services

Your Consent Matters

As a parent, you have the right to make informed decisions about your child's early intervention services. Written consent is required before evaluations begin and before services start. You can withdraw consent at any time.

Types of Consent Required

1. Consent for Evaluation

Before your child can be evaluated, you must provide written consent that includes:

  • Agreement to have your child evaluated
  • Understanding of the evaluation process
  • Areas of development to be assessed
  • Professionals who will conduct the evaluation
  • Your right to decline evaluation

2. Consent for Services (IFSP)

Before services begin, you must consent to the Individualized Family Service Plan (IFSP):

  • Agreement to the services recommended
  • Understanding of each service and its goals
  • Frequency and duration of services
  • Location where services will be provided
  • Your right to decline any specific service

Note: You may accept some services and decline others without affecting eligibility.

3. Consent for Release of Information

Your written permission is required to share information with:

  • Your child's pediatrician or other doctors
  • Other early intervention providers
  • School districts (for transition planning)
  • Any other agencies or individuals

You control who receives information about your child. Consent can be limited to specific information and specific recipients.

Your Rights Regarding Consent

  • Voluntary - All consent is voluntary; you cannot be forced to consent
  • Revocable - You may withdraw consent at any time in writing
  • Informed - You have the right to full information before consenting
  • Specific - Consent is limited to what is specified on the form
  • Time-limited - Some consents expire and must be renewed

How to Get Started

Consent forms will be provided by your service coordinator when you begin the early intervention process. If you have questions about any consent form:

  • Ask your service coordinator to explain
  • Take time to read forms carefully
  • Don't sign anything you don't understand
  • Request forms in your native language if needed
